Your body was built to move — and when it doesn't, everything downstream suffers. Here's what the research actually says, and how to use exercise like a prescription.

We talk about exercise like it's a lifestyle choice — something nice to do when you have the time, the motivation, or the right gym membership. But in functional medicine, we see it differently. Movement is one of the most powerful interventions we have. It changes your hormones, reshapes your metabolism, reduces inflammation, and protects your brain. No medication does all of that at once.

The problem is that most people are either doing too little, doing it in a way that doesn't match their biology, or pushing hard in a body that isn't recovered enough to benefit. This post is about getting the dosage right — because like any medicine, movement works best when it's targeted.



Why Your Body Needs Movement to Function

Movement isn't just about burning calories. At the cellular level, exercise triggers a cascade of repair and renewal signals. It improves insulin sensitivity, which means your cells respond better to blood sugar. It stimulates mitochondrial biogenesis — essentially telling your cells to build more energy factories. It reduces circulating inflammatory markers like CRP and IL-6. And it activates BDNF, a protein that supports the growth and repair of brain cells.

When you sit for long stretches, these signals go quiet. Muscles that aren't used become less responsive to insulin. Inflammation creeps up. Energy production slows. Over time, this is how sedentary behavior becomes a driver of chronic disease — not just a passive absence of health, but an active contributor to it.

When Exercise Isn't Working — or Making Things Worse

If you're exercising consistently and still feel exhausted, gaining weight, or getting sick frequently, the problem usually isn't effort. It's context. High-intensity training on top of poor sleep, chronic stress, and a nutrient-depleted diet can actually raise cortisol further and drive inflammation. Your body reads overtraining as a threat — and responds accordingly.

In functional medicine, we look at exercise in the context of your whole picture: your cortisol rhythm, thyroid function, nutrient status, and recovery capacity. Sometimes the prescription is less intensity, not more. Sometimes it's simply more sleep.

Movement and Chronic Condition Management

Exercise isn't just preventive. For patients with existing conditions, it's often therapeutic. Regular movement has well-documented effects on:

Insulin resistance and type 2 diabetes — Strength training and aerobic exercise both improve glucose uptake independent of weight loss. Many patients reduce their medication burden with consistent exercise.

Thyroid and adrenal health — Gentle, consistent movement supports circadian regulation and can reduce the cortisol load on the HPA axis. Intense daily training, on the other hand, can worsen adrenal dysregulation.

Mood and mental health — Exercise increases serotonin, dopamine, and norepinephrine. A 2023 meta-analysis in JAMA Psychiatry found that exercise was as effective as antidepressants for mild-to-moderate depression in several patient groups.

Brain health and dementia prevention — BDNF levels increase with aerobic exercise and are strongly linked to cognitive resilience. Movement is one of the most evidence-backed tools for reducing dementia risk.
