The Safety Picture Has Changed

Much of the fear around HRT stems from a 2002 interpretation of the Women's Health Initiative (WHI) study that was later found to be significantly flawed and misrepresented. The women in that study were older, already had cardiovascular risk factors, and started HRT many years after menopause—circumstances that do not reflect how HRT is prescribed today.

Current evidence from long-term follow-up studies and a robust body of peer-reviewed research shows that for healthy women under 60 who begin HRT within 10 years of menopause onset, the benefits substantially outweigh the risks. In fact, an analysis of 30 trials involving over 26,000 women found HRT was not associated with increased cancer mortality—and women who start before age 60 actually appear to have a decreased risk of all-cause mortality.

In November 2025, the FDA took the significant step of removing longstanding "black box" warnings from menopausal HRT products, acknowledging that decades-old cautionary language had discouraged appropriate use and failed to reflect modern evidence.
