Your Labs Are "Normal" — But Do You Feel Normal?

Why "within range" isn't the same as optimal — and what your bloodwork might really be telling you.


You went to your doctor. They ordered labs. A few days later the results came back and you got a message — or maybe just silence — indicating everything is "normal." But you're exhausted. Your hair is thinning. You can't lose weight no matter how hard you try. Your brain feels foggy by 2 p.m. every afternoon.


Sound familiar? You're not imagining things. And you're not alone.


At MorningStar Integrative Health, one of the most common things we hear from new patients is: "My doctor said my labs are fine, but I feel terrible." This is one of the central tensions between conventional medicine and functional medicine — and it's exactly what we're going to unpack today.

The Problem with "Normal" Reference Ranges


Standard lab reference ranges are calculated based on the average values from a large population — often the people who get routine bloodwork, many of whom are already managing chronic illness, carrying excess weight, or living on processed food and poor sleep. When your result falls within that range, it simply means you're not an outlier from that population.


That's a far cry from thriving.


Functional medicine takes a different lens. Instead of asking "Is this value abnormal enough to diagnose a disease?", we ask: "Is this value in the range associated with how a healthy, energetic, symptom-free person actually functions?" Those can be very different numbers.

Notice a pattern? In almost every case, the functional optimal range sits in the healthier portion of the conventional range — or even narrows it considerably. A TSH of 4.0 might technically be "normal," but many people feel sluggish, cold, and foggy at that level. A ferritin of 15 is technically not anemia — but it's low enough to cause hair loss, fatigue, and poor exercise tolerance.


The Thyroid: A Perfect Case Study

Thyroid dysfunction is one of the most under-recognized contributors to fatigue, weight gain, depression, brain fog, and cold intolerance — and it's also one of the most commonly missed on conventional labs.

Here's why: most doctors order only TSH. But TSH is a pituitary hormone — it tells your thyroid what to do, but doesn't tell you if the thyroid is actually doing it. To get the full picture, we look at:


A Comprehensive Thyroid Panel Includes


  • TSH— pituitary signal, best in range of 1.0–2.0

  • Free T4— the main hormone the thyroid produces

  • Free T3— the active form your cells actually use

  • Reverse T3— can "block" T3 receptors under chronic stress

  • TPO & Anti-TG Antibodies— detect autoimmune thyroiditis (Hashimoto's)


Someone can have a normal TSH and still have poor T4-to-T3 conversion, high Reverse T3, or Hashimoto's antibodies quietly damaging their thyroid tissue. Without looking, you'll never know — and the patient will continue to be told their thyroid is "fine."


Blood Sugar: Catching the Drift Before It Becomes Diabetes


Conventional medicine diagnoses pre-diabetes when fasting glucose reaches 100–125 mg/dL, and diabetes at 126 mg/dL or above. But blood sugar dysregulation — insulin resistance, glucose swings, impaired cellular signaling — begins years, even decades, before those thresholds are crossed.

In functional medicine, we watch for early warning signs that include:



Early Metabolic Red Flags (Even in "Normal" Range)


  • Fasting glucose trending above 86 mg/dL

  • Fasting insulin above 5–7 µIU/mL (often not ordered at all in conventional care)

  • Hemoglobin A1c creeping toward 5.4–5.6%

  • Triglycerides above 100 mg/dL

  • HDL below 60 mg/dL

  • Uric acid elevation (linked to insulin resistance)


By identifying these patterns early, we have a wide-open window to intervene with nutrition, movement, sleep, and stress management — before a disease label is ever necessary.

Inflammation: The Quiet Culprit

High-sensitivity C-Reactive Protein (hs-CRP) is one of the most powerful predictors of chronic disease risk — cardiovascular disease, autoimmunity, cognitive decline, and more. Yet a result of 2.9 mg/L would be called "normal" in most labs, when functionally, anything above 0.5–1.0 mg/L warrants attention.


Other inflammatory markers we evaluate include homocysteine (linked to cardiovascular and neurological risk), ferritin (which rises with inflammation, not just iron status), and fibrinogen. Together, they paint a picture of how much "background fire" is burning in your body — often long before symptoms become obvious.


Micronutrients: The Most Overlooked Panel

Most annual physicals don't include micronutrient testing at all. But deficiencies in key vitamins and minerals are incredibly common, and they silently erode energy, mood, immune function, and metabolic health.

  • Vitamin D— most people need 50–80 ng/mL; the majority are well below that

  • Magnesium (RBC)— serum magnesium is often normal even when cells are depleted

  • B12 & Folate— critical for energy, methylation, and neurological health

  • Zinc— involved in immune function, hormone balance, and over 300 enzymatic reactions

  • Iron & Ferritin— ferritin below 50 ng/mL is a common hidden cause of fatigue

  • Omega-3 Index— reflects cardiovascular and brain health; rarely checked, frequently low



When ferritin sits at 18 and B12 at 250, a patient might get a "normal" on their report — but they'll be dragging through their days. Replenishing these to truly optimal levels can be transformative.
